If you’re enrolled in one of these Aetna® medical plans, you and your covered spouse can each earn up to $300 in incentive monies for taking healthy actions.

  • HDHP with Aetna Choice POS II Plan
  • Aetna Traditional Choice® Indemnity Plan
  • HDHP with Aetna Traditional Choice Indemnity Plan
  • Aetna International Traditional Choice Indemnity Plan

Note: This program is not available with the Aetna Medicare Advantage with Prescription Drug (MAPD) Plan.

Healthy actions to earn incentives

The resources and screenings below are at no cost to you if you follow the directions provided in the Health Incentives Program materials. For you to receive the incentive monies, you’ll need to complete the actions by December 31 of the plan year, unless otherwise noted below. But don’t wait! Get started today on the path to a healthier you.

Complete an online health assessment — $75

The health assessment is an online questionnaire that covers a wide range of health-related topics — and it takes just 15 minutes to complete.

To start, log in to Aetna.com > Stay Healthy.

Complete an online Journey® — $75 each, up to 4 per year

Your health assessment results will highlight areas where you can reduce your health risks. Select an online Journey to help improve that focus area. It usually takes about a month to complete a Journey.

Complete a biometric screening by November 30 — $150

This simple group of three health checks (a blood test, waist measurement and blood pressure measurement) determines your risk of heart disease, stroke and diabetes. Learn more about the screening and how to schedule it.

Complete 3 calls with a nurse to work on a health goal — $75

Have one or more chronic medical conditions, such as asthma or diabetes? Help is available from the Condition Coaching program. The program provides digital and one-on-one support for chronic conditions. A nurse will help you understand and follow your treatment plan, make healthy lifestyle choices and avoid health complications. You can call 1-866-269-4500 to learn more.

Take your kids for a preventive (well-child) exam — $50 per child

Schedule a preventive exam for dependent children under age 18 to earn $50 per child per year. Be sure to schedule them with an in-network provider for no-cost care.

How you spend your earned incentive monies

If you’re enrolled in the Aetna Choice POS II Plan or Traditional Choice Plan:

  • When you earn your first incentive monies, they’ll be deposited onto a Health Reimbursement Account (HRA) debit card, which will be mailed to you.
  • As you earn more incentive monies during the year, they’ll automatically be added to your debit card balance.
  • You can use those funds to help pay for eligible medical, dental and pharmacy expenses. You can also let the funds build for future expenses.
  • You have until the end of your plan’s claims submission period (April 30) to submit claims for eligible expenses incurred the prior plan year.
  • Any funds remaining on your HRA debit card at the end of the year will carry over to the following year.

If you have a High Deductible Health Plan with an HRA or Health Savings Account (HSA):

  • Earned incentive monies get deposited into your HRA or HSA.
  • You can use the funds for eligible health care expenses for any covered tax dependent (up to age 24 for dependent children).
  • For those with an HSA (active employees and pre-65 retirees), earned incentive monies remain in your HSA until you use them.
  • For those with an HRA, earned incentive monies remain in your HRA as long as you have the HDHP plan with an HRA.
